maintenance log

i was wandering if any one has made a good maintenance log to keep track of all repairs and fluid changes, i was going to make one but having issues with microsoft office right now.

I use Google Docs so I can easily edit it on my phone. I just keep simple info on it: date, mileage, service done, who performed, parts used. Vendor for me would be parts source and/or who performed. Not fancy, but I don't need a lot of details for my purposes.
